Taking the Next Step: Our Step-Down Service Model

At Breason Care Supported Living CIC, we understand that the path to greater independence can be challenging. That’s why we’ve developed a Step Down Service model to support individuals with complex mental health needs as they transition toward a more fulfilling and independent life.

Our services provides a safe and nurturing environment for adults in need of supported living.

Who is This Service For?

This service is designed for individuals who:

  • Are ready to move on from intensive support settings.
  • Need personalised guidance to develop life skills, build confidence, and increase self-sufficiency.
  • Would benefit from a structured and supportive environment as they work toward independent living.

What to Expect?

  • Individualised Support Plans – Our experienced key workers collaborate with each person to create tailored plans that align with their specific needs and aspirations.
  • A Safe and Supportive Setting – Our accommodation fosters a welcoming and inclusive atmosphere where individuals feel secure, valued, and empowered.
  • A Clear Pathway to Independence – We provide structured transition plans to support individuals in gaining independence through employment opportunities, skills training, and access to community resources.
  • Flexible Support Options – Assistance is personalised to each individual, including scheduled daytime visits, evening check-ins, and access to a 24/7 on-call helpline for additional support when needed.
  • Results-Driven Approach – We monitor progress using key milestones, such as enhanced independent living skills, better employment prospects, stronger social connections, and improved financial management.
  • Skills Development Assessment – We evaluate an individual’s ability to manage daily responsibilities essential for independent living. This helps identify strengths and areas for improvement, ensuring they receive the right level of guidance and support.
 

Our Service Users

While we strive to support as many service users/ tenants as possible it is important to know that we are unable to accept tenants/service users with high level drug or alcohol issues or severe mental health conditions that make them harm themselves or others or that lead to violent and destructive behaviours. In accepting a tenancy, the individual agrees that core support will be delivered by Breason Care Supported Living CIC on the tenancy service level agreements.
